We always welcome new patients to our practice! When preparing for your first appointment, please complete the forms below.

Notice of Privacy Practices

In addition to your new patient paperwork, please bring these:

-Current dental insurance card
-Insurance benefits package
-List of all current medications

-Copies of any x-rays taken within the last 5 years.*
We also ask that you arrive 15 minutes early so that we can enter your paperwork into our system.

*NOTE: Typically the x-rays we request are the last Bitewing x-rays and the last Full Mouth Series or Panorex. The x-rays can be emailed to our office at DrsKitahara@comcast.net.
